This is an opportunity for a Band 6 Clinician to join us in our homeless health team!
Summary about the Service
The service provides a clinical community care that tackles health inequalities by working with the most vulnerable groups.
The work is focused on those who are socially excluded, homeless and unemployed and or have substance misuse problems.
The staff provide: General Health advice; Dietary advice; Minor injury care; Advice on safer sex/drug/alcohol use; Help registering with a GP and other health services, Access into IAPT and other Mental Health services.
The Outreach service treats individuals on an ad-hoc basis in both a community and clinic setting in environments where this group of adults meet.Main duties of the job
Day to day, you will contribute to tackling health inequalities by working with the most vulnerable groups. You will proactively engage with individuals who have identified health inequalities at a range of venues in local communities, helping them to improve their lifestyle and overall health.
You will be an experienced practitioner who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
Working as part of the team and as an autonomous practitioner, you will be self-driven demonstrating safe, evidence based clinical decision making to deliver compassion expert care for patients who are homeless. You will work collaboratively with other agencies including health and social care professionals, specialised voluntary staff and Police to meet the needs of patients, supporting the delivery of policy and procedures, providing nurse leadership as required.
The role demands a unique practitioner with exceptional communication skills and an ability to adapt to challenging situations. You will need to quickly develop trust with this cohort of patients, encouraging them to access and utilise and engage with service for their often complex needs.
