Job summary

An exciting opportunity has arisen for a band 6 heart failure specialist clinician to join our community-based service. We are looking for a dynamic clinician to join our well-established service that covers Bristol, North Somerset and South Gloucestershire (BNSSG)

Our community-based specialist service works to improve outcomes for patients living with heart failure. By working as part of a wider Multi-Disciplinary Team we strive to deliver best evidence care that is individualised for our patients. We work closely with our partners in Primary and Secondary Care, providing early treatment intervention and supporting the transition from hospital to long term management. The service also offers advice and guidance to Primary and Community Care.

You will see patients in community-based clinics and in their own homes and therefore a full UK driving license is essential or ability to be able to travel independently to fulfil the role.

The services core hours are currently Monday - Friday 0830-1700

There will be an element of working across BNSSG to meet service delivery, but we look to deliver clinical caseload elements of the service within geographical locations with bases currently within Bristol and North Somerset

The team is supported by a Consultant Cardiologist

Main duties of the job

The post holder will manage a caseload of patients with complex Heart Failure, using evidence-based practice and patient centered goals to assess, plan, implement and evaluate interventions.

To provide expert support, advice, and education to raise awareness of the needs of patients with Chronic Heart Failure.

To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues.

Job responsibilities

Your primary purpose will be to support the Sirona BNSSG Community Heart Failure Service; working with senior clinicians in the delivery of care throughout BNSSG.

The role will support the delivery of Key Performance Indicators within the service specification for the BNSSG Adult Community Services contract working in partnership with other BNSSG providers.

You will be responsible to the team leads and be supported in the planning and delivery of high quality, safe care.

Service Overview:

The BNSSG Heart Failure team provide specialist Heart Failure support encompassing:

  • direct care to patients including providing personalised planned interventions to support people to remain safe, well and as independent as possible in their own homes;
  • education and training to health care professionals across the pathway and
  • developing the focal point for responsive, high quality clinical advice in the out of hospital system, closely integrated with secondary care specialities ensuring a seamless service.
  • The team will work at a local level with the Community teams supporting the Primary Care Networks to ensure that Specialist Advice and Support Services (SASS) are delivered locally to the population and their needs

Expectations

The Post Holder is expected to;

  • To work closely with Senior Heart Failure nurses and team leads to effectively manage heart failure patients in the clinic setting and at home.
  • Assist in the management and delivery of the diagnostic and nurse led follow-up clinics in the community setting.
  • Provide excellent decision-making skills, demonstrating higher levels of judgement.
  • Initiate, adjust and monitor therapy under the guidance of the supervising physician by using the agreed protocols of care.
  • Support primary health care teams to achieve the standards for heart failure in line with NICE /ESC and other key guidelines.
  • Detect and act appropriately to patient problems experienced due to their condition or treatment by utilising excellent clinical examinations skills.
  • Review patients in nurse-led community heart failure clinics. Provide support through home visits for housebound patients.
  • Liaise with community and secondary care services regarding on-going care for patients with heart failure in the community, providing emergency advice and support as required.
  • Assess, monitor and evaluate outcomes of care and service provision in both acute and stable situations
  • Assist in the management of end of life patients, liaising with members of the MDT to ensure all patient and carers needs are met
  • Contribute to setting standards of care and auditing clinical outcomes and service provision.
  • Work with the Integrated Network Teams, primary, secondary and social care and other community providers to support care, promote integrated working, maximise opportunities to facilitate patient discharge and support admission avoidance by providing timely specialist support in community settings.
  • Work in a genuinely integrated fashion with primary, community and secondary care specialist services to deliver comprehensive support to both the patients and wider Specialist pathway.
  • Support senior team and ensure effective delivery of service; adhere to all policies of the organisation by prioritising, organising and delegating appropriately.
  • Assist with the development, implementation and evaluation of initiatives to provide a high quality of individualised patient care.
  • To participate in operational activities such as quality monitoring, objective setting, Service meetings and clinical supervision
  • To be willing to work flexibly across BNSSG to support service delivery
  • Work alongside the operational manager to ensure the delivery of key performance indicators and targets.
