Job summary

An exciting opportunity has arisen to work at our Rehabilitation Unit at South Bristol Community Hospital.

The unit consists of 30 beds, which provides a safe environment where individuals who are medically stable can receive rehabilitation assessment with focused therapy and support from our multidisciplinary team, to support independence before they return home.

We are looking for Nurses to work with service users who have been recently discharged from an acute hospital setting who require care and rehabilitation following acute admission.

Main duties of the job

To assist multidisciplinary healthcare professionals in the delivery of individualised holistic care which is delivered with compassion, dignity and respect.

Work with the Multidisciplinary team and contribute to the prevention of harm by carrying out risk assessments such as nutrition, pressure area care, continence and falls.

To adhere to the medicine management policy, patient group directives and other relevant procedures and guidelines to ensure safe preparation and administration of medicines.

To safely and accurately record physiological observations and escalate appropriately as per trusts policy.

Assist patients to mobilise safely using equipment and techniques prescribed and recorded in their plan of care.

Promote a culture of ambulatory care by supporting patients to be as independent as possible with washing, dressing in own clothes, encouraging mobility from admission and building health promotion into care as appropriate.

To take responsibility and support new staff and student nurses or NVQ/apprenticeship students allocated to them.

To assist senior staff in the management of the unit and be responsible for the unit in the absence of senior staff. This includes maintaining a safe level of staffing and making appropriate decisions regarding admission and discharge of patients.
