Job summary
NHS@Home is an
innovative service, providing care for people who are acutely unwell in their own
homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative
to hospital and helps people return home from hospital more quickly, too.
The service uses a
mixture of face-to-face visits, phone and video calls, and remote monitoring through
digital technology, to help people recover safely in the comfort of their own
home.
NHS@Home has six
pathways, covering a wide range of conditions including frailty, respiratory,
heart failure, and general surgical recovery. Our team provide a very wide
range of acute healthcare treatments within the home, in addition to monitoring
health conditions and vital signs for deterioration.
The service is run in
partnership between Sirona care & health, North Bristol Trust, University
Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

being discharged into the care of the NHS@Home respiratory service made the

Main duties of the job
Working in the NHS@Home team, you'll contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people.
- Working within the
NHS@Home service, your duties will include:
- Providing acute care to patients with complex
and specialist needs in the community, across Bristol, South Gloucestershire
and North Somerset.
- Working alongside the multidisciplinary team to
develop, implement and evaluate care plans.
- Assessing the health and psychosocial needs of
the patient within the hospital and community setting.
- Plan and implement treatment and use a wide
range of service to meet their needs.
- Utilising a wide range of clinical skills to
support the ongoing management of patients in their home environment e.g.
delivering IV therapies, venepuncture, cannulation and wound care (not
exhaustive list).
- Communicating effectively, networking and
liaising to ensure high quality responsive, safe, efficient and compassionate
care is provided for our patients, carers and families.
The post holder will work in collaboration with the NHS@Home team at
Sirona care & health and BNSSG system partners at NBT and UHBW. Hours of
operation are 0800-20:00
You must hold a valid full UK driving license and have access
to an insured car with business insurance. Travel expenses will be paid.

- Work as part of the clinical and administration team to ensure that all tasks are completed and enabling safe, timely care
- Deliver direct care for patients including ongoing basic clinical assessment of an acutely unwell patients in the community, venepuncture
- Work with senior clinicians and medics as part of the MDT to develop safe & effective treatment plans for people within the @Home pathway
- Provide monitoring and assessment of patients in the @Home pathways , advising on care and treatment and reporting changes in patient condition as required in line with local protocols and guidance
- Support senior staff within the Virtual Ward to complete tasks efficiently and effectively
- Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes
- Educates and advises patients regarding treatment and when they should contact a doctor/111 or 999 in line with guidance
- Plan and provide the management of remote @Home pathways ; working as part of an integrated team and in partnership with other health care providers.
- Where appropriate, liaise directly with the patients GP or Consultant or other HCP teams to support treatment planning.
- Make referrals to other health care professionals or agencies where appropriate.
- Maintain effective communication and feedback to the original referee.
- Ensures the privacy and dignity of all patients is maintained
- Enable others to promote equality and diversity in a non non-discriminatory culture
- Supporting colleagues and responding to requests from administrative and healthcare professionals, working effectively within the wider team.
- Takes responsibility for ensuring compliance with all relevant practice policies/guidelines, e.g. safeguarding, resuscitation, confidentiality, data protection, health and safety.
- Works in accordance with the professional guidance provided by the NMC / HCPC HCPC.
- Ensure effective delivery of service and adhere to all policies of the organisation by prioritising, organising and delegating appropriately.
- Deputise in the absence of senior staff
- Support staff who may be bank in the delivery of the service through supervision and support
- Exemplify standards and behaviours for the service whilst using remote tools such as MSTeams
- Contribute to setting standards of care and auditing clinical outcomes and service provision.
- Work alongside the operational manager to ensure that the service is compliant with CQC essential standards and any other relevant regulatory bodies
