Job summary

This is an exciting opportunity to hold a key position within the community. You will play a vital role within the Sirona INT, providing Band 5 care. There are plenty of opportunities within Sirona and this could be the next step in your bright career.

You will be working within our integrated network team (INT) delivering elements of clinical interventions to patients in their own homes across nursing and/or therapy care depending on your skills set. This can include skills in wound and catheter care, equipment provision, support with activities of daily living, exercise programmes, venipuncture etc.

Each INT is made up of:-

Advanced Clinical Practitioners (ACP) - Long Term Conditions

Clinical Lead Therapist

Community Nurses

Allied Health Professionals including Occupational Therapists and Physiotherapists

Nurse Associates, Assistant Practitioners

Highly skilled support staff

Administrative staff

You will receive a huge amount of support whilst working with the rest of the INT to provide safe and effective care to patients across our services which include planned nursing, planned therapy, hospital discharges and urgent care response.

Main duties of the job

  • Contribute to the holistic assessment of service users.
  • Deliver clinical care delegated by the registered healthcare professional, in accordance with a person centred plan of care.
  • Undertake the appropriate training supported by a robust competency framework to carry out the requirements of the role and a deliver a competent clinical service.
  • Continuous maintenance of competencies following policy.
  • Work within your level of competence, only performing tasks you have been trained to do and feel competent to carry out
  • Escalate any changes in patient presentation or concerns to a more senior member of your team.
  • Assist clinical staff in the assessment and treatment of patients referred to the INT.
  • Supporting patients with personal and domestic living activities, rehab and exercise support, provision and adaptation of basic mobility and functional equipment, basic wound care and venepuncture.

  • Take every opportunity to support and educate service users and their carers to promote independence, good health and personal safety.
  • Provide and maintain a safe and comfortable environment and seek further advice from trained staff if an emergency situation should arise
  • Contribute to the movement and handling of individuals to maximise their comfort and safety.
  • Identify risks and act accordingly to minimise risk to self, service users.

  • NVQ Level 3 in Health & Social Care or equivalent or willingness to work towards

Experience
