Job summary

An exciting opportunity has arisen to work at our Sub-Acute Stroke Rehab Unit! We are looking for an experienced Nurse to join the team.

The unit (SSARU) consists of 30 beds at South Bristol Community Hospital, which provides a purpose built rehabilitation environment where individuals who are medically stable can receive rehabilitation assessment, along with focused therapy and support from our multidisciplinary team, to support independence before they return home.

We are looking for Nurses to work with patients who have been recently discharged from an acute hospital setting who require care and rehabilitation following stroke.

This is a great steppingstone into an ACP or Leadership role. You will be working within a supportive environment, where we are committed to our ongoing clinical development and further study.

Main duties of the job

Senior nurse on site responsible for implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs after their acute stroke.

Provide leadership and guidance to more junior nursing and healthcare staff in day to day situations, development of their clinical skills and initial point of escalation for clinical deterioration.

Use clinical reasoning skills to assess, plan, implement and evaluate patient centred intervention programmes.

Support and oversee the patients 24hr health and care needs, including complex post-stroke medications and feeding regimes.

Support the patient and wider therapeutic workforce in achieving the patients rehabilitation goals.

Carry out nursing interventions, drawing on a wide range of specialist knowledge gained from practical experience and specific training.

Work within a multi-disciplinary team with guidance from senior nursing colleagues and medical colleagues to provide safe and effective stroke specific care for service users with a range of post-stroke health and care needs

  • On the NMC register
  • Member of Royal College of Nursing and/or evidence of other professional liability insurance to practice
  • Evidence of CPD and commitment to lifelong learning

Desirable

  • Stroke specific training or learning
  • Up to date knowledge of Evidence Based Practice, local and national standards in particular for Stroke management

Experience

Essential

  • Experience of working with stroke service users at an acute, or sub-acute level
  • Effective communication skills, written and verbal and non-verbal including regular experience of working with patients with stroke related communication deficits.
  • Experience of supervising staff and students of own profession

Desirable

  • Experience of working in a rehabilitation environment and supporting a 24/7 rehab model
