Job summary
Would you like to make a real difference to the health and wellbeing of children, young people and families? An exciting opportunity has arisen for a part time Community Nursery Nurse to join the Yate Health Visiting team in the South Gloucestershire Public Health Nursing locality.
Sirona Public Health Nursing service is currently undergoing an exciting transformation to embed the i-THRIVE Framework. This facilitates a preventative/early intervention approach to service provision which empowers children, young people and their families to be actively involved in decisions about the support they receive.
The health visiting service is a workforce of specialist community public health nurses who provide evidence based information, support and interventions to families with children in the first years of life, and help empower parents to make decisions that affect their familys future health and wellbeing.
You will work within Yate Health Visiting Team as part of a skill mix, supporting the delivery of integrated, evidence based services for children and their families, with a focus on prevention, promotion and early intervention and improving health and reducing health inequalities.
You will promote a holistic and sensitive approach to supporting parents with their childrens social, emotional and developmental needs. You will work in a combination of family homes, childrens centres, and other community settings.
Main duties of the job
To support the delivery of the Healthy Child Programme, antenatally and for children from birth to 5 and their families, as delegated and supervised by the Community Nursery Nurse Lead.
To support the facilitation of the Baby Hubs
To deliver the community nursery nurse 9-12 month review
To deliver specified Support Packages to families, which are short term, time limited interventions.
To liaise effectively with families, health professionals and Early Years Settings as part of the co-ordinated approach to communication within the locality.
To follow processes to ensure that work is focussed and time limited and outcomes are shared with the named health visitor for the family.
To engage in effective reflective practice, continuous professional development and supervision in order to cultivate the skills, knowledge and resilience to meet the needs of service users and the health visiting contract.
