Job summary
This is an exciting time to join
the Sirona care & health Heart Failure Service as a Senior Heart Failure Specialist
Clinician. You will be working within
the Bristol, North Somerset and South Gloucestershire (BNSSG) community service
as we build on existing models of care and introduce new ways of working in
line with transformation plans and further integration with system partners.
You will demonstrate highly
effective clinical experience and expertise in supporting patients with cardiac
conditions and with Heart Failure. This
includes providing clinical expertise underpinned by enhanced skills in
Clinical Practice, Facilitating Learning, Leadership, and Evidence Research and
Development.
You will provide support and
guidance to patients across BNSSG in a range of settings such as clinics and
home settings.
As a Specialist Service we work
closely with other health providers across the interface, as well as to other
services within Sirona. The service delivers
an advice and guidance service, and education to health care professionals.
There will be an element of
working across BNSSG to meet service delivery but we look to deliver clinical
caseload elements within geographical locations, with some remote working
opportunities to support efficient delivery of care.

Main duties of the job
The post holder will deliver high quality care within the heart failure pathway and provide expert clinical practice and advice to patients and support to staff. This is delivered in a range of community and primary care settings.
This will include providing clinical expertise, underpinned by the advanced skills within their speciality developed in Clinical Practice, Facilitating Learning, Leadership, and Evidence, Research and Development
Taking a key role in the development and delivery of the Heart Failure Service:
- Through an Expert Clinical Heart Failure role in conjunction with the Clinical and Operational Leads within the service
- Working collaboratively with other members of the multi-disciplinary team and key stakeholders to establish a whole systems approach in order to deliver improvements in healthcare for people with heart failure
- To develop and deliver Health Care Professional training around the subject area of heart failure
