Job summary

An opportunity has arisen to work in collaboration with our Specialist End of Life Service in Sirona!

You would be welcomed into one of our busy nursing teams to support service users who have nursing needs in their own homes as well as having the opportunity to spend part of the role focusing on palliative and end of life care, providing an opportunity to maintain a wide range of community nursing skills and have a focus on palliative and end of life care.

Initially, the palliative and end of life element of the role is as a 12 month pilot, and we hope that this will become a permanent element of the role. However, please note that the band 5 post is a permanent post within the INT, and should the pilot come to an end the community nursing aspect will continue on a full time basis.

Full support, training and orientation will be provided for both aspects of this Band 5 role.

Main duties of the job

You will have the opportunity to develop additional competencies within the requirements of the multi-disciplinary approach to the INT and also as part of a specialist end of life service.

Due to the diversity of need in our locality, this is a brilliant opportunity to develop your skills through training and in-team support in a range of nursing interventions, along with dedicated time to focus on palliative and end of life care.

Essential

  • BSc or equivalent experience in adult nursing or an Allied Health Professional role
  • Current NMC or HCPC registration
  • Evidence of continuing professional development and commitment to lifelong learning
  • Working knowledge of clinical outcomes measures, clinical audit
  • Knowledge of the principles of Clinical Governance and its application

Desirable

  • Up to date knowledge of Evidence Based Practice, local and national standards

Experience

Essential

  • Effective communication skills written, verbal and non-verbal
  • Able to adapt communication for individuals with communication difficulties
  • Able to function effectively as a team member
  • Able to make clinically reasoned judgements when working with a variety of conditions
  • Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff
