Job summary
This is a flexible role with the post holder working as part of the Urgent Care Pathway in the Community and as part of the clinical team of the inpatient rehabilitation unit at South Bristol Community Hospital providing clinical leadership and escalation.
Community Pathway:The primary function of the ACP Urgent Care is to visit service users in their home within 2 hours of receiving an urgent referral and provide appropriate clinical care to prevent avoidable acute hospital admissions working closely with the Integrated Network Teams and Specialist Advice and Support Services in health and social care, or to facilitate urgent referrals to secondary care where this will benefit the individuals clinical outcome.
Inpatient Rehabilitation Unit:The ACP will be clinically responsible for people who have been recently discharged from an acute hospital setting who require a short period of rehabilitation at South Bristol Community Hospital in order to return to their usual place of residence. The unit is a community based unit providing active rehabilitation and ongoing nursing for those people being supported on Discharge to Assess Pathway 2.
Main duties of the job
Following a hospital stay, the ethos for all people being discharged is Home is Best and a large number of people are supported to return home utilising the robust and established Discharge to Assess Pathway 1. During their stay, those people we support will experience therapy intervention in our multidisciplinary unit, with support given to them, their carers and families around complex discharge planning. This will enable a timely and safe discharge to the persons usual residence or to a more suitable location if appropriate.
We actively promote a multi-disciplinary model on the Unit to ensure the person we are supporting is kept at the heart of every decision.
As an advanced practitioner you will provide a service that focuses on achieving and demonstrating overall improvements in health care for people who require urgent care. You will also provide clinical leadership and direction to the INT, supporting the Locality Manager and Assistant Locality Manager to deliver excellent evidence based clinical outcomes. To provide organisational and service resilience, you may be expected to cross cover other teams or services, both within the locality and across Sirona services.
