Job summary
Are you an enthusiastic ACP or do you have an aspiration to
become one as the next step in your career development?
Can you evidence your commitment to the four pillars of advanced
practice?
An exciting opportunity has arisen at our Stroke Sub-Acute Rehab Unit (SSARU) at South
Bristol Community Hospital which will improve stroke care regionally. As
part of this development,we are looking to recruit ACPs to provide specialist
stroke care. Our ACPs will be working alongside Speciality Doctors and Stroke
Consultants to provide high quality clinical care to our service users to
support them through the sub-acute stage of their rehab and recovery journey
post stroke.
Our SSARU meets the needs of
people who are unable to return home immediately after a hospital stay as they
need specialist rehabilitation. The unit consists of 30
beds at SBCH, which provides a safe environment where individuals who are medically stable can receive rehabilitation assessment, along with therapy and support from our multidisciplinary team, to support independence
before they return home. Once the person
arrives home, their stroke care is handed over to our Integrated Community
Stroke Service (ICSS) at Home team.
This is a fantastic opportunity for you to be part of our
exciting new model of care from the beginning with opportunity for cross
organisational working as part of the one Stroke workforce in BNSSG.
Main duties of the job
Across Bristol, North
Somerset and South Gloucestershire, health, care and voluntary sector partners
are working together to improve services for people affected by stroke. This
includes work to improve the prevention of stroke, as well as to provide the best
possible stroke emergency care, rehabilitation and care at home once
individuals are discharged from hospital.
Our vision is to provide seamless,
multidisciplinary care, so local people have the best opportunity to survive
and thrive after stroke.
If you are currently working
at Enhanced Care Practitioner (ECP) level, we would offer this role as a Band 7
with the support and training to complete your level 7 Masters in Advanced Practice within 3 years or as
required to become an 8A ACP with a clinical speciality of Stroke.
There will be
another SSARU unit in Weston supporting 12 beds and is currently run by United
Hospitals Bristol and Weston (UHBW).
Please see attached the job description for further and more in-depth details about the role.
