We are looking for a Full Time Health Care Assistant to join our Responsive Services Team. The teamwork, a variety of shifts between 8am-8pm 7 days a week.

This exciting role offers the opportunity to work in a caring, supportive, and professional environment where you will contribute to the care and support of patients in the community.

Your role as a Health Care Assistant will be varied and interesting, working with our Virtual Ward and Urgent Community Response Teams, ssupporting patients with early discharge from hospital and avoiding unnecessary hospital admission.

You will assist in the planning of the patients care, engaging with the patient, family, and carers in their own home through implementation of individualised care plans.

Candidates will need to be able to work autonomously and as part of a team, travelling to patients homes in town and rural settings. The post-holder will be supported by senior clinicians, to carry out tasks listed below.

You must be a licensed car driver and have access to a car for this post.

Main duties of the job

Carrying out baseline observations

Calculating NEWS score, escalating deterioration

Monitoring patients weight

ECGs

Venepuncture

Fitting equipment such as toilet seats, commodes, walking frames

Monitoring patients skin integrity

Supporting patients with personal care

Monitoring and supporting patients with nutrition and hydration needs.
