We are recruiting band 4 Nurse
Associate (NMC registration) to join our expanding community teams in East Surrey,
based in Redhill.
You will be
part of a highly motivated and supportive district nursing team delivering safe
and effective NHS care to patients in the community.
In return, we

continuing professional development and revalidation.
You will be
supported by the Clinical Team Lead for the Community Network and your line
manager to develop your skills and will work independently to support
development of the service and implement evidence-based practice.
You will provide
nursing care for a wide range of conditions to people in their own home and
residential settings, and you will be supported by a manager, team and mentor
to fulfill your role.
Main duties of the job
You will be responsible for assessing,
planning, implementing, and evaluating individualised care for patients under
the direction of a Clinical Team Lead and District Nurse.
You will work in close collaboration with
health and social care professionals within the Primary Care Network to prevent
your patients being admitted to hospital.
You will provide advice and support to
junior staff and students in the delivery of effective, high-quality care.
Due to the nature of the role,
it is essential that you are able to drive, have a car suitable for business
