Job summary

An exciting opportunity has arisen for an experienced clinical pharmacist to join our friendly pharmacy team working on the ward at Caterham Dene Hospital.

You will work in an exceptionally friendly clinically led team which is genuinely patient-centric and encourages innovation that results in real improvements in quality care and positive patient experience.

Within the role the successful candidate will be working with the wider multidisciplinary team of nurses, therapists and doctors.

There will be learning opportunities and support for the post holder to undertake the non medical prescribers course and to develop their full potential.

This role is full time, Monday - Friday 9am-5pm.

Main duties of the job

1.Support the Head of Medicines Management (HOMM) in developing the specialist clinical pharmacy service to wards and the community team, in line with local and national standards and policies.

2.To deliver specialised pharmaceutical advice to other healthcare professionals, including participation in multidisciplinary and clinical governance meetings.

3.Support other healthcare staff in meeting risk management by highlighting risk to HOMM, financial management and clinical governance responsibilities around the use of medicines.

4.Support and mentor ward-based assessments of placement and work experience students, medicines management technicians and pre-registration pharmacists.

5.To carry out clinical pharmacy audit and practice research in conjunction with the Head of Medicines Management and to review First Community policies and procedures.

6.To provide leadership and manage medicines related tasks and incidents for the ward and community teams.

Essential

  • Masters degree in pharmacy or equivalent
  • Postgraduate Diploma in Clinical or Technical Pharmacy or equivalent demonstrable experience
  • Registered as a pharmacist with The General Pharmaceutical Council

Knowledge

  • Specialised knowledge of the treatment of patients especially older people
  • Knowledge of how the delivery of the clinical pharmacy service must meet the needs of local and national priorities
