We are looking for Rehabilitation Assistants to join the Responsive Services team in First Community Health and Care. The team is made up of the Intermediate Care and Urgent Community Response Teams.
The Rehabilitation Assistants work collaboratively with the multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses, other Rehabilitation Assistants and Administrative Staff. The Intermediate Care team provides short term care packages and rehabilitation to patients who have recently been discharged from hospital or those at risk of being admitted to hospital. The post of Rehabilitation Assistant involves providing care visits and rehabilitation in the patients own home under the direction or our nurses and therapists. The Intermediate Care Team works closely with the Urgent Community Response team and this role offers the opportunity to work across both teams to support as required.
Due to nature of the role it is essential that applicants are able to drive and have a access to a car suitable for business use. The use of public transport to complete this role is not possible due to the area that we cover.

Main duties of the job
Central to this role are:
- An interest in working in rehabilitation of patients in the home setting to improve patients independence and achieve their goals
- Experience of working autonomously and as part of a team
- Must be able to manage time effectively
- A willingness to undertake competencies to enhance skills in the role.
- You must be a car driver and have access to a car
- The service operates from 08:00-20:00 7 days a week, 365 days a year the post holder will be expected to work on a rota basis.
