We can offer flexible working hours across a seven-day week so that you can achieve a work/life balance.

You would work within Responsive Services, which comprises of the Intermediate Care team, Virtual Ward, Urgent Community Response, Referral Hub and Integrated Discharge Team, supporting patients to facilitate discharge and support health and care needs of patients within their own homes.

Due to nature of the role it is essential that applicants are able to drive and have a car suitable for business use.

All staff undertake an induction programme. At First Community Health and training and development opportunities are supported through an annual appraisal process and in-house training.

The service operates from 08:00-20:00 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis.

Main duties of the job

Central to this role are:

The role will deliver a range of health and care interventions to patients who are being discharged from the hospital, this will include support with a therapy/rehabilitation care plan and practice support with everyday activities like washing and dressing. There patients will have a variety of need and will be discharged along a number or different pathways including Home First and Virtual Ward. There will also be patients who are supported with care at home as an alternative to being admitted to hospital.

Day to day work will be varied and will be in the persons own home, or where they usually live. You will need your own transport and be happy to travel around the east Surrey area.
