Job summary
This is an exciting opportunity for a registered Staff Nurse to join the award-winning Inclusion Health team (under the direction of Band 7 Specialist Homeless Health Visitors) to deliver the Healthy Child Program and health advice, support and signposting, via assertive outreach to homeless and asylum-seeking families and Gypsy, Romany and Traveller communities in Surrey.
The role requires an experienced Band 5 registered nurse to support and work collaboratively within the skill-mix team. Applicants will require the skills and experience to deliver targeted care and advocacy for vulnerable families who are experiencing homelessness in a range of settings, as well as supporting members of the team with safeguarding and outreach work. We encourage secondments from your current post to experience ground-breaking nursing care in a community setting.
This role is fixed term until 31 March 2025.

Main duties of the job
The successful applicant will support the team in the delivery of community health support to vulnerable families via telephone, virtual and face to face contacts, as required.
The role will involve working as part of the Children and Family Health Inclusion Health Team, across Surrey, delivering the Healthy Child Program through targeted outreach and providing a trauma-informed approach. Previous experience working with these disadvantaged client groups would be advantageous but training, including cultural awareness, will be offered. Experience with line managing, delivering community projects and public speaking, including public health messaging and delivery would be advantageous, as well as safeguarding experience but training and development opportunities will be offered. Regular psychological support is provided to support the team and individual wellbeing is prioritised with team away-days, flexible working and some dedicated time to work from home for record-keeping and online training.
This exciting initiative involves targeted, assertive outreach to communities experiencing multiple barriers to healthcare and aims to reduce both health inequalities and poor health outcomes.
