Job summary

This is an exciting opportunity for a band 5 community staff nurse to join an expanding Responsive service team where acutely unwell patients can be cared for in their own homes. This role would be suitable for both experienced and newly qualified nurses as part of our Preceptorship Programme.

Responsive services allow patients to receive the care they need at home safely and conveniently rather than being in hospital. The multi-disciplinary service works 7 days a week from 8am 8pm.

You will work in a caring and supportive environment under the supervision of the virtual ward matron closely with our urgent community response, intermediate care and virtual ward teams.

You will support the individuals within their home environment with all aspects of care including daily living, providing person centred care and promoting health and independence through awareness raising and care navigation to other professional as appropriate.

You will need to be able to work autonomously and as part of a team. To support you within your role, we have an excellent learning and development to help further develop your clinical skills.

You must be a licensed car driver and have access to a car for this post.
