We are seeking a band 6
Community Sister to join the Community Nursing Neighbourhoods.
We anticipate the successful
candidate will have the opportunity to complete the District Nursing course
to gain a specialist practitioner qualification in district nursing.
We are
looking for caring, compassionate community nurses who have a wide range of
clinical skills and are keen to develop their career in the community. In
return we are committed to supporting you to develop your leadership,
management and clinical skills.
You will need to be forward thinking, motivated, enthusiastic, and
able to work collaboratively with patients, their carers and health and
social care colleagues.
Main duties of the job
Working as part of a team, you will be responsible for supporting the
Team Lead with the management of a caseload of patients, working with other
health care professionals across the Neighbourhood aiming to avoid hospital
admission.
You
will need to have a flexible approach to working some weekends and bank
holidays. Although this role is advertised
as full time, we will consider part-time working arrangements.
Due
to the nature of the role, it is essential that you are able to drive, have a
car suitable for business purposes, and a valid driving licence.

accountable, without direct supervision, for the delivery of a high standard of
cost-effective nursing care in the community. You will be responsible for the
continued management of a caseload of patients, working within a neighbourhood
providing care closer to home aiming to avoid hospital admission. You will
ensure that patients receive safe, individualised and effective standards of
care by assessing, planning, implementing and evaluating episodes of care.
You will work as
part of a highly skilled multidisciplinary team, working closely with patients
and carers, linking with acute hospitals and primary care, actively encouraging
effective communication at all times with all relevant agencies.
You will promote
and develop evidence-based practice in line with current policy and procedures
and NMC code of conduct.
You will be
responsible for nurturing the development of the team and your direct reports.
The service
currently operates 365 days a year between 8am and 6pm and you will be required
to work shift patterns between these times.
