Job summary
We
are looking for an experienced neurological Occupational Therapist to join
our established Community Stroke and Neurological Rehabilitation Team.
Our service provides multi-disciplinary
assessment and rehabilitation for patients with a wide variety of
neurological conditions across east Surrey, delivering care in patient homes,
outpatient clinics and community venues.
Our clinicians work within smaller teams to cover different aspects of
the service, including our Early Supported Discharge (ESD) pathway for
Stroke, Long Term Conditions community service and neurological outpatient
clinics.
The
successful candidate will work closely with Team Leads from other therapy
disciplines, Clinical Psychology and Stroke Nursing to ensure the successful
operational management and ongoing development of our dedicated neurological
rehabilitation team.
Central
to the role are strong communication and organisational skills, proven
specialist clinical skills in neurological rehabilitation and management of
long-term conditions and experience of line management and clinical
supervision.
Some
Saturday working may be required on a rota basis.
You must be
a car driver and have access to a car for work purposes.

Main duties of the job
- To work jointly with Team Leads from other disciplines within CSNRT to coordinate daily team management and triage of referrals, ensuring that appropriate assessments, onward referrals and outcome measures are completed, in line with national and departmental targets.
- To autonomously manage a complex clinical caseload at an advanced level of specialism.
- To work closely with all Team Leads, Clinical Leads and the Service Manager within Long Term Condition Support Services (LTCSS) to promote joint working, monitoring of standards and quality improvement work.
