An exciting new Practice
Development Nurse role has been developed in Responsive Services in First
Community Health and Care.
Responsive Services
comprises of Urgent Community Response (UCR), Intermediate Care Team (ICT),
Virtual Ward and Referral Hub. The teams focus is preventing unnecessary
hospital attendance.
The ICT are an
admission avoidance and facilitating discharge service, who provide assessment,
short-term care and rehabilitation in the patients home.
UCR are an
admission avoidance team, responding within 2 hours to acutely unwell patients
in the community to prevent hospital admissions.
Virtual Ward supports
the provision of hospital level care to frail patients in their own home under
the guidance of the acute frailty consultant.
The Practice Development Nurse role will enhance nurse practice through clinical expertise, education and leadership focusing on delivering quality care and outcomes for patients.
The position requires an experienced nurse who uses evidenced based care to create consistent practice across the teams.
Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.
This role is a secondment for 12 months.
Main duties of the job
As part of the Responsive Services leadership team you will support staff with the management of daily activity mapping core nursing to the correct skill mix. You will provide a structured development programme to junior staff supporting with comprehensive assessments in the patients home, focusing on supporting patients to manage their health and wellbeing. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home and minimize repeated admissions.
The candidate will
be able to adapt to change and work in a busy environment. Excellent communication skills are essential
to enable liaison with all team members and wider supporting services.
You will also provide professional leadership and line management for
designated staff and will have a role in the leadership of the wider
multidisciplinary team.

The service runs
from 8am-8pm 7 days a week, 365 days a year, with the post holder working flexibly
to support the service.
