Job summary

We are recruiting a band 5 staff nurse for our Community Night Nursing Service.

In this role, based at our community hospital ward, you will provide nursing care for a wide range of conditions, prioritising community patients in their own home. When not on visits, working with patients at the community hospital.

The service hours are 10.30pm 06.30am. All community visits are undertaken together with a Health Care Assistant. This role is for 30 hours per week, although 22.5 hours per week would be considered.

We are looking for a caring and compassionate nurse, who would like to develop their skills further. In return we are committed to supporting you in your continued professional development.

Main duties of the job

You will be responsible for attending planned visits or those necessary resulting from direct contact from the patient or their relative. You will be responsible for assessing, planning, implementing and evaluating individualised care for existing patients on the district nursing caseload either planned or unplanned.

You will be responsible for communicating with the community teams via existing processes.

When not on visits you will support ward patients. Caterham Dene Ward is a 28 bedded physical rehabilitation ward for mainly older adults.
