Community Nurse Band 5
CSH Surrey
The Integrated Neighbourhood Teams are looking for a Band 5
Community Nurse to join our community nursing team across the neighbourhoods of
Ashford & Stanwell, Staines, Sunbury & Shepperton, and Egham &
Virginia Water.
The Community Team
visit patients in their own home as well as care homes. We offer excellent
opportunities for training and development of both your clinical and leadership
skills. The team has experienced Senior District Nurses who will ensure that
you have the necessary support for your personal development. The community
nursing service works over a 7-day week 8.00am to 8.00pm offering a variable
shift pattern and includes weekend and bank holiday working.
This post requires an
Adult Nursing qualification. Newly qualified nurses are encouraged to apply for this position.
You will be
supported with a comprehensive induction and training package as needed, to
help you settle in, with the time and opportunity to get to know the people on
your caseload, along with development opportunities.

Job responsibilities
Purpose
Responsible for the delivery of care and for managing
service provision in accordance with national and local priorities,
organisation policies, procedures and guidelines and in accordance with the
Nursing and Midwifery Council.
Principal duties to include:
Participate in
assessment and care planning of patient care in conjunction with the District
Nurse
Implements
and evaluates care and treatment in agreement with the patient and as defined
in the care plan escalates any deviation from the care plan or change in
condition to the District Nurse / GP accordingly
Communicates
effectively with patients, carers and all members of the multidisciplinary team
Prioritises
daily work in line with clinical / patient need
Maintains
accurate and legible patient records in line with professional and
organisational requirements this includes contemporary paper records and
electronic patient record system
Implements
and works to ensure evidence based care / best practice
Actively
participates and contributes to team meetings and daily handover
Initiates
health promotion activities with individuals, patients and carers
Takes
responsibility for a defined caseload in the absence of the District Nurse,
under the direction of the Practice Teacher / Team Leader
Actively
involved in the development and implementation of community nursing service
developments and initiatives
Participates
in clinical audit and the implementation of changes to ways of working /
clinical practice as a result of findings, recommendations and action plans
Participates
in risk assessment and promotes the safety and well-being of patients, staff
and any other persons at all times and assist in ensuring a safe work
environment

complaints, untoward incidents, accidents and areas of clinical risk are
identified and reported to the District Nurse in accordance with the
organisation policies
