Job summary
This is an exciting opportunity to support adults with complex neurological conditions who live within NW Surrey. As part of service transformation we have recognised the need for an autonomous decision maker that can work across services to provide a link to local charities and the voluntary sector and other key stakeholders. As a new post, the successful applicant must have the necessary skills and be willing to support the senior leadership team with developing metrics audit and service reviews.

Main duties of the job
The post holder is responsible for the specialist care provision for patients in NWS with a progressive, life-limiting neurodegenerative disorder for which there is no curative treatment. This will include people with Huntingtons disease, and Motor Neurone Disease (MND)
To co-ordinate care and services for patients to ensure that they and their carers receive the highest quality of care, support and experience within agreed protocols and allocated resources, together with other members of the multi-professional team. This includes working with colleagues from neurology, Respiratory, Speech and Language, Dietetics, Palliative Care Community Neuro Physiotherapy & specialist Community Neuro Occupational Therapy, third sector partners, including the social care services, hospices, and charities such as the Motor Neurone Disease Association (MNDA).
To work collaboratively to develop expertise and resources to provide sound clinical advice and judgement to patients and their families, and other health and care professionals.
To provide home visits where patient assessments are carried out, treatment plans are made, advance care planning is discussed, and using professional clinical judgement, appropriate investigations or referrals are undertaken.
To monitor local provision against relevant best practice guidance, working as a colleague within Community Services INT and with neurological specialists across Surrey Heartlands Integrated Care System (ICS).
