Job summary
We are looking for an Assistant Practitioner to join our Surrey wide Childrens Integrated Continence Service, based at Leatherhead Clinic. This is a full-time permanent post, Monday to Friday, job share and flexible hours also considered.
You will work as part of an experienced, motivated skill-mixed team providing evidence-based continence and advice and support to promote continence to children ages 4-19 years. This includes continence assessments, providing advice and support and reviewing progress for children and young people. The service is delivered in a variety of settings Surrey wide supporting children and young people including home visits, schools, special schools and other community settings. Telephone support is also part of this service. The role includes working in partnership with health colleagues, education and other agencies to promote continence.
We provide in house and external training to develop your skills within the community setting and within childrens continence to support the children and young people in Surrey.

Main duties of the job
Working within the Childrens Integrated Continence Service to support children with a variety of continence issues including daytime enuresis and constipation, in various community settings.
The role involves assisting and supporting with the identification / assessment of childrens continence needs, carrying out assigned tasks for children, young people and families and delivering evidence-based care interventions under the supervision (direct and indirect) of a registered practitioner.
Principal duties include:
1. Contributing to assessment, planning, implementing and evaluation of individual continence care interventions in a variety of community and clinic settings.
2. Completing noncomplex continence assessments with children and families
3. Working with other team members to provide advice to children and young people (CYP) and carers in relation to continence needs including assessing for suitable containment products.
4. Acting as a resource and role model to others involved in continence care
5. Contributing to the development, delivery, and evaluation of the Childrens Continence Service education programme
6. Maintaining accurate, clear, concise and contemporaneous records and communicating information to members of the Team as necessary.
7. Liaising with external agencies with regards to follow up and providing feedback to registered practitioners as appropriate
