Job responsibilities

To take responsibility for their adult learning and to inform their Practice Teacher and University contact should there be any exceptional circumstances preventing the completion of any aspects of the course.

Maintain confidentiality, it is stressed that much of the information which is handled by the student nurses is of a highly confidential nature and should not be divulged to another party nor should it be discussed with colleagues unless it is essential to do so.

To follow organisational sickness and absence policy, reporting sickness or absences to both the employer and university.

Undertake duties as allocated by practice teacher

Work collaboratively with the skill mixed health visiting team to ensure equity of service provision, promoting good practice and the sharing of resources.

Participate in supporting good working relationships with Family Centres, midwifery, GP practices and the wider multi-disciplinary team.

Where appropriate work with Children and Family Health Surrey (CFHS) Nurse prescribing lead to prescribe in line with local and national policy guidelines.

Make Every Contact Count with children, and their families in a variety of community settings e.g home, children centres, nurseries

Participate in the delivery of the Healthy Child programme 0-5

Participate well child clinic sessions within agreed venues and children centres.

Participate to identify and support children and families who have unmet health needs, providing support and targeted interventions and where necessary referring to specialist services.

Ensure the changing needs of individuals and groups are identified in a timely manner and ensure adjustments to programmes of care are made Take appropriate action in cases of suspected or actual child abuse, working within Surrey safeguarding childrens code of practice and procedures

Attend child protection conferences/core groups as required with practice teacher or agreed 0-19 health care professional

Ensure that clear concise reports are written and that organisational record keeping guidelines are adhered to

Participate in clinical audit and evaluation of service including Support and attend group clinical supervision as per organisation policy Participate in meetings as appropriate

Be prepared to be flexible within contracted hours

Work in accordance with the needs of the service, undertake other duties as required within current skill set

Communication skills (factor 1)

Demonstrate effective communication networks with clients and other health professionals, statutory and voluntary agencies

Participate in designing, producing and presenting health promotion sessions to differing sizes of groups

Be competent in writing concise accurate contemporaneous client records using approved CFHS templates in line with NMC guidelines.

Be competent at making referrals to the wider multi-disciplinary team e.g. MASH Participate in producing child centred reports for children who are subject to safeguarding plans or CIN/LAC reviews

Participate in discussions at professional forum and team meetings

Analytical and judgemental skills (f3)

Be able to participate in working with complex family situations and responding and referring appropriately

Work towards undertaking health needs assessments, using child centred approach to gather health information that identifies any unmet health needs for children and families.

Participate on team/service audits

Planning and organisational skills (f4)

Be responsible for planning and maintaining up to date work diary

Support Health Visitor initiatives to deliver health promotion activities and community based public health awareness events.

Prioritise diary to meet needs of families

Maintain compliance with statutory and mandatory training

Patient / client care (f6)

Support and adhere to organisation including infection control policy

Promote uptake of immunisations: make every contact count

Holistically assess children and families

Work within clinical pathway framework and own level of competency

Promote an environment that is conducive to children and familyengagement

Be aware of Health Visitor key performance indicators and ensure associated client contacts are accurately recorded for reporting purposes within the team

Support I want great care initiatives to acquire feedback from service users.

Policy and service development (f7)

Work within CFHS policies and guidance

Contribute to service and policy review and development of health visiting protocols and guidance

Provide statistical data to support commissioning requirements.

Have an awareness of and participate in clinical audits

Financial and physical resources (f8)

Responsible for care and safe keeping of medical devices, organisational property and ethically manage organisations resources

Identify when equipment or disposable resources need replacing and alert clinical team lead .

Human resources (f9)

Post Reference

Maintain NMC registration in line with revalidation requirements and abide with NMC Code of Professional Conduct.

Participate in the setting of personal objectives with Practice Teachers through the job appraisal and clinical supervision processes

Provide DBS documentation in line with organisational procedure

Information resources (f10)

Maintain record keeping in line with NMC and organisational guidance

Ensure safe storage and access to IT systems to protect confidentiality in line with organisational Information Governance policy

Research and development (f11)

Contribute/lead on organisational audits

Support evidence based practice and disseminate learning to wider health visiting team members

Freedom to act (f12)

Work to organisational policy and procedures

Work under direct and indirect supervision of Practice Teacher

Qualifications

Essential

• Applicants must be a Registered Nurse (RN1) or NMC Registered Midwife, with an active NMC PIN at the time of applying for the programme.
• Applicants must have a BSc Hons 2:2 or above.
