Main duties of the job
Deliver packages of care to support families both in the home and clinic settings. make contact with children and families in a variety of community settings e.g. school,
home, children centres, participate in the delivery of universal 0-19 HCP developmental reviews using ASQ
questionnaires as appropriate or directed. Support school screening under direction of SCPHN school nurse. Undertake continence assessments and reassessments as directed by school nurse. Participate in Child Health Clinics. Build and develop good working relationships with childrens centres, school and
wider multi -disciplinary team. Support families in relation to infant/ child nutrition in the clinic, home or other setting in
line with Baby Friendly Initiative. Carry out home visits to selected families to teach play skills and promote early
learning through demonstration and shared activities. Contribute to health promotion activities under the direction of SCPHN health visitor and
school nurse e.g. home visits, groups, community events, display boards. Assist in running children and family focused groups e.g introducing solids. Take appropriate action in cases of suspected or actual child abuse, working within own
competencies and Surrey safeguarding code of practice and procedures and participate
in safeguarding supervision.

transport vaccines and clinical waste to and from immunisation sessions safely. Take appropriate action in cases of suspected or actual child abuse, working within own
competencies and Surrey safeguarding code of practice and procedures and participate
in safeguarding supervision Take an active part in clinical supervision. Contribute to nursing student experiences in facilitating opportunities to shadow at
health contacts. To maintain clear and comprehensive, signed and contemporaneous records
according to company procedures. To assist with clerical and administrative duties as required. Assist in service audits. Participate in meetings as appropriate. Participate in own PDR appraisal. Be prepared to be flexible within contracted hours. Be skilled at adapting language and professional approach to a varied client group. Be proficient in presenting health promotion sessions to differing sizes of groups, eg
parents and children. Be competent in writing concise accurate contemporaneous client records using
approved CFHS templates. Be competent at making referrals to the wider multi-disciplinary team.Able to understand complex family situations and respond and refer appropriately
escalate concerns to SCPHN. Be able to undertake health screening, using child a centred approach to gather
health information that identifies any unmet health needs. Contribute to population profiles within locality and school communities. Participate in team/service audits.Be responsible for planning own work diary to meet caseload allocated by SCPHN

- GSCE Maths and English (or equivalent) NVQ level 3 qualification in
- childcare/child development.
- Relevant experience of working with children, young people and families, and new mothers Previous experience of working as part of a team.
- Understanding of confidentiality particularly in relation to children and their families.
- Good organisation and time management skills
- Ability to work independently within agreed guidelines and policies
- Able to develop, establish and maintain positive relationships with other both internal and external partner agencies
