Main duties of the job

Working as a member of the multi-disciplinary team you will undertake holistic assessments, clinical diagnosis, and treatment of patients in their own home, who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions, support complex hospital discharges and maximise their independence.

As a result of your assessment, you may also arrange short term support provided by our Senior Community Rehab Assistants, and liaise with colleagues in local GP practices, locality frailty hubs, adult social care teams, and other community services.
