Main duties of the job

As a Diabetes Specialist Nurse, you will:

  • Conduct advanced clinical assessments and create personalized management plans for your patients.
  • Interpret clinical signs, laboratory results, and other functional measures to initiate appropriate treatments and interventions.
  • Make autonomous, safe, and effective decisions to enhance patient health outcomes.
  • Communicate complex and sensitive information to patients, their families, and carers with empathy and clarity.
  • Organize and prioritize your workload, with flexibility to meet service needs and uphold quality standards.
  • Lead and mentor other healthcare professionals, enhancing their skills and contributing to community and primary care development.
  • Drive the adoption of evidence-based clinical practices and develop specialist pathways within the service.

The CSH Diabetes Specialist Nursing team provides specialist clinical advice and care, supporting the delivery of all commissioned activities to adult patients within the North West Surrey area and delivering an integrated model of diabetes services.

The role of the Diabetes Specialist Nurse service is to promote the health and wellbeing of people with diabetes enabling them to achieve optimum glycaemic control through informed self-management. The post holder will provide a link between GP and consultant led specialist teams and will provide education and support both to people with diabetes and those who care for them.

The post holder will be responsible for facilitating improved diabetes care through the provision of advisory services, including telephone, video and face to face support within practices, clinics in an acute setting, nursing homes and patients' own homes.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service.

  • A minimum of 5 years experience as a Registered Nurse.
  • Evidence of working within a multidisciplinary team.
  • Experience in managing patients with diabetes, including both type 1 and type 2.

Desirable

  • Research or clinical audit experience.
  • Community healthcare experience.

Skills

  • The ability to clinically assess, deliver, and evaluate high-quality care.
  • You must be able to work autonomously, reflect on your strengths and development needs, and work under pressure while balancing multiple priorities and meeting deadlines.
  • Demonstrateable evidence-based practice in line with clinical governance principles.
  • A willingness to develop new skills appropriate to the role is required.
  • Confident in using information technology
  • Valid UK driver's license with access to private transport for business purposes.
