Job summary
The Urgent
Community Response team supports patients who are housebound by providing
nursing assessment and care in their own homes to avoid hospital admission, 24 hours a day. We are looking for an experienced motivated, reliable Nurse to join our Urgent Community Response service to provide day and night cover for patients throughout the North West Surrey area.
Main duties of the job
Working in partnership with specialist community services and other
social care teams, to provide high quality nursing care and advice. The service
offers professional advice, support and skilled nursing care to enable patients
to live as independently as possible in their own homes and avoid unnecessary
hospital admissions. It is important to note that the post-holder may be
required to rotate between community services including Urgent Community
Response. There may also on occasion be a requirement to work days.

Principal
duties to include:
- Work as part of the team undertaking lead responsibilities as
negotiated and agreed with Out Of Hours coordinator
- Treat all patients as individuals, respecting their privacy and
dignity at all times
- Assess individual care needs, implement and evaluate holistic
programmes of care
- Involving, supporting, informing and educating family/carers
- Take every opportunity to promote health and wellbeing of the
patient
- Develop effective communication and liaison systems
- Maintain clear and comprehensive, signed and contemporaneous
records according to CSH Surreys procedures
- Facilitate/undertake training and professional updating for team
and students
- Act as an assessor and mentor to qualified and unqualified staff
- Maintain, develop and monitor standards of evidence based nursing
care
- Maintain own professional and clinical integrity in line with NMC
guidelines
- Support/mentor team members and participate in clinical supervision
activities
- Work without direct supervision and to maintain close liaison with
the senior team member, deputising as required
- Be responsible and maintain timely patient care
- Be responsible for maintain data entry and other record keeping
- To work in co-operation and partnership with all other
professionals and agencies involved in the care of the patient
- Be aware of and act upon when necessary, procedures that are in
place to protect vulnerable individuals
