Band 3 Childrens Community Carer

Do you have a passion for supporting children/young people with additional health needs? Have you previously worked with children in a health, educational or social care setting?

An exciting opportunity has arisen to provide clinical care to children/young people with additional health needs (including tracheostomies and ventilators) at home

  • Minimum of 22.5 hours per week
  • Flexible working to include night shifts 10pm-7am, weekends and bank holidays
  • You will need access to a car with business insurance to travel to the family homes within Surrey
  • Previous experience is desirable however not essential as full competency training is provided; there will be an initial induction period followed by supervised practice

Job description

Job responsibilities

Purpose

  • To assist in the delivery of care activities in the child/young persons own home and in the community following an intensive training programme and competency assessment. Staff will work unsupported following assessment with the support of Clinical Case Manager. Participates in the induction and development of new staff, under the direction of the Clinical Case Manager.
  • To be flexible to work unsocial hours, including night duty over a 24 hour period, 4/5 week rota pattern including weekends and Bank Holidays

Principal duties to include:

Communication skills (factor 1)

  • Establishes and maintains effective verbal and written communication with colleagues, children/young people, families and members of the multidisciplinary team, providing clinical information as required. There may be barriers to understanding.
  • Acknowledges the boundaries of the role,
  • Maintains Professional working boundaries with the families and colleagues at all time
  • Raises any issues/concerns with the Clinical Case Manager relating to the childs clinical care, workload and responsibilities.
  • Uses work mobile telephone and work email accounts appropriately, maintaining regular contact with the team and responding in a timely manner.
  • Uses the team On Call system appropriately out of hours.

Analytical and judgemental skills (f3)

  • Responsible for identifying changes in patients conditions and taking appropriate action, documenting and reporting to Clinical Case Manager.

Planning and organisational skills (f4)

  • Effectively identifies all clinical equipment and disposables are in place to ensure safe delivery of patient care
  • Any shortfall or concerns should be documented and reported to the core team or out of hours where appropriate

Physical Skills (f5)

  • Ability to use keyboard and computer
  • Ability to complete competency workbooks including tracheostomy, ventilator and gastrostomy care
  • Required to develop skills to change of tracheostomy breathing tubes, replacement of Percutaneous Endoscopic Gastrostomy (PEG) feeding tubes etc.
  • Required to have developed skills in the use of various equipment that requires accuracy within a narrow margin for error e.g. ventilators, oxygen saturation, enteral/nasogastric/nasojejunal feeding pumps etc.

Patient / client care (f6)

  • Responsible for working with children/young people who are dependent on others for meeting some or all of their health and wellbeing needs.
  • Ensure the care that is delivered to the children on the caseload is of a consistently high standard
  • Responsible for discussing the childs/young persons support plan with the qualified nursing team, ensuring understanding of their role in providing care to meet the needs, including an understanding of the Safeguarding of Children.
  • Responsible for delivering care in accordance with relevant care plans, escalation plans, Standard Operating Procedures and guidance.
  • Required to administer medication following Medicines Management Training and having been deemed competent by a Clinical Case Manager.
  • Responsible for caring for highly technologically dependent children/young people.
  • Responsible for assessing the child/young persons condition in relation to the child/young persons support care plan and escalation plan.
  • Ensure any concerns relating to the child/young persons clinical condition, safety or in relation to safeguarding concerns are escalated and reported.
  • Understands service issues to enable problems to be identified and solutions recommended.
  • Able to prioritise work to meet child/young persons care needs.

  • Complies with company policies and implements these where appropriate
  • Contributes to development of local policies and Standard Operating Procedures

Financial and physical resources (f8)

  • Ensures at the start of each shift adequate supplies are in place and uses resources appropriately ensuring minimal wastage
