Job summary

An opportunity has arisen for a registered nurse to join our friendly and supportive team at The Rosemary Foundation.

We are a charity providing hospice at home care and services over a 24-hour period to patients with a life limiting illness. Supporting their families, carers and friends to enable patients to be able to die at home.

We work closely with other health care professionals, supporting primary health care teams and the Macmillan palliative care team at Midhurst.

You will use your nursing skills to assess, plan and evaluate care using evidenced based practices which meet the holistic needs of the individual.

As a member of the team you will support colleagues, providing clinical leadership to Health Care Assistants, student nurses and volunteers

You will have experience of working in the community and caring for palliative care patients, you will be competent and confident caring for patients and families in their own homes, including care homes.

Main duties of the job

Provide palliative care to patients, families and their carers in their own living environment.

To support patients with palliative or end of life prognosis and their families/carers to achieve their preferred place of care.

In collaboration with internal colleagues, external referrers and stakeholders, provide Palliative and End of Life Care for patients within a 15 mile radius of Petersfield.

Work as an effective team member, organising, managing and prioritising own workload.

Manage a caseload of patients supporting with telephone calls, home visits, and bereavement support and symptom management.

Maintain confidentiality and data protection regarding all personal information and hospice activity, operating within current legislation.

The RGN will be expected, under supervision, to deliver high standards of personalised care and identify needs of the patients through holistic assessment.

The RGN will use their experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care, and medication administration.

They will work collaboratively with the Community Specialist Palliative Care Team (CSPCT), GPs and district nursing teams.

They will monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care.

The RGN will take responsibility for their own continuing professional development and performance, including identifying own development needs.

About us

The Rosemary foundation is a nurse led Hospice at Home charity providing palliative and end of life care to patients in their own homes and support to their families within a 15 mile radius of Petersfield and covering 700 square miles in East Hampshire.

All care and services are provided free of charge. The Rosemary Foundation is funded by donations, community fundraising and legacies.

We work closely with GP's, community nursing teams and specialist palliative care teams to ensure a high standard of end of life care to patients.

Led by the Senior Leadership Team, the team consists of registered nurses, healthcare support workers, administration support and volunteers.
