Walking
With The Wounded (WWTW) supports those who have served in the Armed Forces,
empowering them to regain their independence, thrive and contribute in our
communities. WWTW recognises that for some ex-service men and women the route
to care looks complex and often inaccessible. By providing an integrated care
coordination service working alongside the NHS as part of Op Courage, WWTW will
ensure these individuals are provided the breadth of care they need, as well as
providing support to their families.
WWTWs Care Coordination Service
involves the assessment of needs for your client, the sourcing of appropriate
support and the ongoing navigation of that support. You remain the central
point of contact as you support your client to address their challenges, which can
include a lack of secure housing, debt, social isolation, relationship
breakdowns, lack of education and training, welfare issues or substance misuse.
We are looking for a Support Care Coordinator
who fully appreciates the patience, resilience and self-motivation required to support a dispersed and diverse
caseload of complex individuals. You must be able to work independently,
particularly in managing your time and caseload, whilst recognising that you
are one crucial element of a wider team of support who will need to work
collaboratively to get the best outcome for clients. Experience of working with
veterans would be beneficial, although not essential.
Main duties of the job
To deliver face-to-face care
coordination to veterans with complex welfare issues. To work closely with
colleagues in the NHS Op Courage veterans mental health service to ensure
longevity of support for your clients.
To engage with local healthcare providers and key support
service organisations who will assist in identified support, and create
essential pathways for those to access the appropriate services.

work with veterans to provide rounded support, alongside NHS mental health
services.

Receive
referrals primarily from NHS veterans teams and report back on progress with
service users.

To
undertake holistic assessments, working with the service user, to design a
personalised care plan; identifying support needs to ensure maximum engagement
in improving health and well-being.

To
provide service users with continuity and a coordinated experience of care,
remaining the pivotal point of contact throughout the individuals journey.

To
support the service user through the housing process.

To
establish and maintain effective liaison with stakeholders including health,
voluntary, social and education resources, attending relevant meetings as
necessary.

To
work in partnership with all voluntary and community organisations to build a
comprehensive database of local resources.

Work
effectively alongside colleagues who provide support to the veterans and their
families.

Ensure all reporting
requirements are met.

To
maintain records and adhere to confidentiality, information sharing protocols
and provide monitoring information as required.
