Job summary

We currently have an exciting opportunity for a Veterans Liaison Support Officer (VLSO) to join a team providing support to ex-armed forces personnel. You will be home-based, with a caseload spread across the Northwest, which includes Greater Manchester, Merseyside, Cheshire, Lancashire and parts of Cumbria.

We are looking for a dedicated individual who will empower ex-service personnel to regain their independence after a period of Mental Health (MH) Crisis, supporting them to thrive and contribute to our communities. This role will join a team of existing VLSOs across the North of England as part of a wider collaborative team within NHS Englands Op Courage - the Veterans Mental Health and Wellbeing service, and will work with other service charities and local support services. You will be home based with daily travel around your region to support your clients and build relationships in the community, whilst working as part of the wider regional WWTW and NHS clinical teams.

Main duties of the job

As a Veterans Liaison & Support Officer (VLSO) Enhanced Pathway you will work with those Veterans, who have been referred and accepted into the programme. This work will be home based and coordinate with your Integrated Regional Service, with daily travel around the region working intensely with a small case load of beneficiaries for a short period of time (usually 12 weeks), to empower them to connect and engage with local services, reducing their stress whilst navigating their Mental Health journey. Work may sometimes be required to be scheduled for outside of typical office hours (9-5pm). Each client will have been clinically assessedprior to acceptance onto the programme, and within an MDT type environment you will support formulation of a care plan, which you will work towards with each client.

Engaging with local healthcare professionals, as well as key support service organisations who will assist in identified support, you will create essential pathways for our clients to access the appropriate services and move forward with their Mental Health journey, as per their care plan.

It could be a requirement of the post, that the appointed candidate will hold an honorary contract with the NHS and must pass their recruitment process alongside WWTWs.The appointed candidate will be expected to uphold the standards and values of both organisations.

Work collaboratively in a team made up of NHS (within an honorary contract), Community Support Services, other Third Sector Organisations; empowering and supporting Veterans who have entered the programme into successful outcomes.

To analyse individuals needs and create a support plan that will break down barriers for a successful stabilisation and navigate the pathway of their MH journey.

Develop and maintain a network of organisations who will work with and support the needs of our beneficiaries.

Undertake holistic client assessments, individually and as part of a collaborative.

Work with the beneficiaries, their families and carers within the collaborative, to design a personalised support/care/development plan, identifying support needs to ensure maximum engagement in improving health and well-being.

Provide continuity and a co-ordinated experience of support, remaining point of contact throughout the individuals journey.

Establish and maintain effective liaison with key stakeholders including health, voluntary, social and education resources and attend relevant meetings as necessary.

Work in partnership with all voluntary and community organisations to build a comprehensive database of local resources.

Understanding and applying safeguarding principles and reporting.

Ensuring all reporting requirements are met as directed by the team manager.

Work in accordance with the principles of the General Data Protection Regulations (GDPR) and WWTWs suite of information management and security policies to ensure the security, confidentiality and integrity of client data and information.

Ensure that the programme meets WWTW professional standards and the reporting requirements.

To maintain and enhance personal skillsets; undertake relevant training and development as and when identified. This may include training in specialist areas of support such as trauma awareness, or developing an understanding different therapeutic avenues.

Support the Op Courage partnership with any training/promotion and programme development support required as directed by the programme manager.

Reinforce WWTWs values by behaving in a manner that strengthens and optimises the organisations performance.

Attend regular daily/weekly/monthly regional team meetings as and when required, exploiting technology to assist you where possible.
