Job responsibilities
Purpose Of The Job
To support the nursing team in meeting the
complex medical, health and welfare needs of our students. To act
independently in the absence of a nurse following the appropriate medical
procedures and where applicable to contact outside agencies. All relevant
information must subsequently be relayed back to the nursing team as a
priority.
Main Duties
1.
To be responsible for carrying out direct personal
and clinical care for highly dependent children in both an educational and
residential setting in accordance with the students individual care plan. To
include:
a.
Put up gastric feeds after a Registered Nurse has
checked and confirmed that the tube is correctly sited, ensuring that the
nutritional needs of the student are met in respect of artificial feeding.
b.
Stoma care
c.
Administer prescribed suppositories, enemas, apply
dermatological medicines, and administer nebulisers, naso-gastric aspiration,
and simple dressings as necessary.
d.
Administer agreed medication as instructed by the
Registered Nurse and to report any acts or omissions in respect of medication
to the nursing team.
e.
Be conversant with current emergency resuscitation
techniques, both in theory and practice.
2.
To support the complex medical, health and welfare
needs of students, both within the school day, working closely with the
teacher, the nursing team and other professional staff and external agencies as
required
3.
Ensure consistency is maintained across the Centre,
i.e. between the education, therapy and residential departments.
4.
To undertake and record student observations of
temperatures, pulse, respirations, blood pressure and urinalysis as instructed
by the nursing team.
5.
Undertake pressure area care using relieving
patches and devices as advised by the nursing team.
6.
To work under the instruction/guidance of
nursing/teaching support programmes to enable access for learning for students.
7.
To act as a team member. Communicating relevant
information to the teacher, the nursing team and external agencies as necessary
in relation to the students needs.
8.
To work with the family and other members of the
multi-disciplinary team to help meet and review the students needs. To provide relevant information as required
for meetings held in relation to the ongoing care needs of the student whilst
maintaining student confidentiality.
9.
To provide a high standard of cleanliness and
comfort for the students, ensuring their accommodation is clean and tidy,
including the restocking of equipment as required.
10.
Ensure that the privacy and choice of students is
maintained and respected at all times, with special regards to confidentiality.
11.
Be aware of particular medical needs of the
student, observe and report on their condition or concerns, alerting any
changes/abnormalities to the nursing team and in the absence of a nurse to
carry out the appropriate checks and alert the relevant professionals as
necessary.
12.
To develop own skills and knowledge in relation to
the complex care needs of the dependent student e.g. ventilation, tracheotomy
care, gastrostomy care and use for feeding. To be able to carry out all these
tasks independently having received the correct training and support.
13.
To act within a level of competency and attend
training and update sessions as required.
To identify gaps in knowledge and skills to the Head of Nursing.
14. Demonstrate
physical dexterity and accuracy to undertake specified clinical procedures.
15. To assist in play
and social opportunities for the student in a variety of settings.
16. Help and encourage
students to become as independent as their abilities allow and therefore enable
them to reach their full potential.
17. Be available for
students to talk to and know when to involve senior staff.
18. Follow the
procedures as laid down in the Safeguarding Guidelines and Policy of the
school.
19. To write clear,
accurate and legible reports of care given and individual students progress
and needs as well as contributing to the nursing care plans, ensuring all
documentation is signed and date in accordance with the Centres policies and
procedures.
20. Co-operate with
the Livability staff Supervision and Appraisal programme.
21. To ensure safe
practice at all times whilst maintaining standards by working to the students
individual Care Plan and all relevant Clinical Guidelines.
22. Carry out any
other duties, broadly in line with the nature of this post, as requested by the
Senior Management team.
23. To carry out all
duties in a manner which is consistent with Livability policies, procedures and
practices, and to maintain at all times the Christian objectives and ethos of
the organisation.
Total Communication
To be acquainted with Makaton
signing, use of symbols and VOCAs and use it to support Total Communication
with all students, with due respect to the Communication Charter.
Confidentiality
Any
information relating to people using the service acquired in the course of duty
must be treated in strictest confidence and must be discussed only within the
confines of the work setting with the appropriate members of staff.
Any
information relating to staff acquired in the course of duty must be treated in
the strictest confidence and must be discussed only with other senior staff
members or with the line manager.
Health and
Safety
The post holder must
be familiar with Livabilitys health and safety policy and guidelines. All work should be undertaken so as to be
consistent with these, and so as to ensure own health and safety, and that of
others affected by their work.
