Job Summary (Description of main purpose of job):

To support the Service in providing high standards of care and quality of life. In particular to provide high quality support and promote independence in all aspects of daily life to people in their own homes, in training, in employment and in leisure activities.

Main Duties & Responsibilities

Please note this is a general description for a Support Worker role, but duties are personalised to the Service Users needs.

Personal support:

To develop and sustain warm and trusting relationships with the people we support, promoting their self-esteem, happiness and emotional health.

To respect peoples right to privacy and to ensure that their dignity and individuality is maintained and respected at all times.

Encourage and support people in expressing their needs, views and concerns and enable them to make choices and decisions and to participate as fully as possible in planning and decision making processes.

To support people in developing a socially valued lifestyle which includes a varied range of culturally and age appropriate experiences, building on the persons strengths, interests and aspirations.

Facilitating social activities, e.g. visiting family/friends, walks, cinema, meals out, visits to local amenities/ attractions etc. in line with the individual interests of the person and with their support plans.

To support people in general housekeeping, such as cooking, domestic tasks, preparing meals, laundry and shopping.

Help as required with all aspects of personal care, e.g. showering, dressing, and where necessary managing continence issues with both dignity and respect.

To create opportunities for the people you support to develop and maintain friendships and personal relationships, encouraging maximum participation and inclusion within their communities.

To achieve a range of appropriate communication techniques in accordance with the individual needs of the person.

To provide support to people in their contact with healthcare specialists, social services, housing departments and other external agencies, e.g. attending relevant appointments, helping to explain information, and completing relevant paperwork in partnership with individuals.

To ensure that people receive all necessary advice, care and regular health checks to maintain their physical health and well-being. To promote nutrition, relaxation, exercise and a healthy lifestyle.

Where relevant and when agreed with appropriate Team Leader to support people in planning and taking holidays (including travel abroad), in accordance with the relevant policy and procedures of the organisation, and in line with their expressed desires and personal support plans. Where relevant to act as an escort for the people on holiday working in line with agreed guidelines around assisting people during their vocation.

Where relevant, to encourage and support people in undertaking training and employment opportunities.

Assist or supervise people as necessary with their medication including administering medication, ordering, storage and recording in line with departmental policy and training.

At all times to work within the ethos and values of the Brothers of Charity Services, to develop and operate best practice and to constantly promote and support peoples independent living and participation in their community.

Individual Support Planning

To help as part of the care and support team to develop and update individual support plans which cover daily routines, health action plans, likes and dislikes and key contacts etc.

To ensure daily learning logs are completed and that key communication tools, such as the diary, are updated in a clear and professional manner, avoiding breaches of confidentiality and any use of slang terms or demeaning terminology; and to ensure that all communications are consulted at the beginning of each shift.

To contribute to the support of individuals with the annual person-centred reviews, liaising with relevant personnel (e.g. Family, Friends, and Social Workers) and ensuring, in conjunction with the relevant team leader, that all preparatory paperwork is completed and any actions and minutes taken are agreed and implemented.

Where requested to act as a lead support for individuals and participate in both internal and external planning and review e.g. commissioning reviews, person centred planning etc.

To support people who may express their frustrations and needs through challenging behaviours by using appropriate strategies, management guidelines and intervention frameworks as specified by their Person Centred Plan and support agreements.

To be responsive to the individual needs of people we support within the framework of their Person Centred Plans and to respond flexibly to changing needs.

Financial

To assist people with the understanding and operation of their own benefit and personal banking arrangements in accordance with the policy and procedure and to support people in claiming their full benefit entitlement.

To assist tenants in supported living properties to understand and adhere to the terms and conditions of their tenancy agreement ensuring that household charges and payments are met, and that they enjoy the rights and responsibilities of being a tenant.

To accurately record all personal expenditure according to organisational policy, and be aware and fully compliant with the Services financial procedure at all times.

To ensure all mandatory and other required records are maintained in a timely, correct and ordered manner, e.g. the recording of expenditure and receipting of said and the completion of relevant weekly accounts records, cross-referencing with individual and household bank statements as required.

To submit accurate working time records on time, and in line with departmental guides.

Liaison, Communication and Teamwork

To attend and participate in team and tenant meetings as required.

To actively participate in training, supervisions and staff development sessions, accepting and providing constructive feedback.

Work effectively as part of a team and also work independently on own initiative.

Help with the induction and training of new staff, students and volunteers when requested.

To ensure effective communication takes place between the people we support, team colleagues and line management, through informal or formal channels as appropriate. Ensure regular verbal and written reports are provided to colleagues.

To ensure that ALL relevant communications sent to the property in which you work are shared with the tenants in an appropriate manner that they are given all relevant information and updates.

To assist the Team Leader in rota planning and shift cover as requested, including liaison with the Staff Resources Co-ordinator regarding cooking casual/bank staff for unfilled shifts.

To ensure at all times that confidentiality is maintained with respect to information relating to individuals and the organisation.

To be familiar with, and comply with, all policies and procedures of the service, particularly with those related to supported living. To observe all written policies, procedures and guidelines for good practice agreed on by Brothers of Charity Services.

To report any untoward incidents, accidents, illness or other occurrences with may affect the health or safety of the people we support or employees in accordance with the laid down policies and procedures.

To carry out all duties in a healthy and safe manner in accordance with the services policy and procedures on Health and Safety, including manual handling, risk assessments and food hygiene. Report to line manager any potential health and safety risks identified.

In accordance with Health and Social Act 2008, to actively participate in the prevention and control of infection within the capacity of the role.
