Job summary

The role of the Nursing Associate is to work with the multi-disciplinary team to assess, plan, organise and deliver care in line with the service and patient needs under direct or indirect supervision. You will act within your professional boundaries, providing care for patients form initial history taking, clinical assessment and treatment under the care of the practice.

The Nursing Associate has a broad knowledge base across the lifespan and fields of nursing, providing holistic and person cantered care and support of people of all ages and in a variety of settings. The Nursing Associate works independently under the leadership of the practice nurse/ANPs/GPs. They are equipped with the knowledge, understanding, skills attitudes and behaviours relevant to a nursing associate and will work to a nationally recognised NMC code of conduct and meet the NMC standards of proficiency.

Main duties of the job

An exciting opportunity has arisen for a Nurse Associate to join our growing multi-disciplinary team. The post will be hosted by Bishops Close Medical Practice, but you may be required to work on behalf of all 5 practices in the Sedgefield North PCN Enhanced Access service. The working pattern may include an 8pm weekday finish and 9am-5pm Saturday working occasionally.

Job responsibilities

Clinical Responsibilities of the Nurse Associate:

  • Deliver high quality, compassionate care under the direction of a registered care professional with a focus on promoting health and independence
  • Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual
  • To work on PCN wide initiatives e.g. Enhanced Access, vaccine clinics; flu and COVID.
  • Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a registered care professionals
  • Work without direct supervision, at times delivering care independently in line with the individuals defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed
  • Provide person-centred care and promote health and independence through awareness raising and care navigation to other professionals as appropriate
  • Perform and record clinical observations including blood pressure, temperature, respirations, pulse
  • Undertake clinical tasks including cannulation, venepuncture, ECGs, and cytology
  • Undertake Chronic Disease management reviews
  • Monitor and support the practice nurse with the management of patients with long term conditions using a patient centred approach
  • Recognise changes in the patients condition which requires the intervention of others and understand when to refer to senior practitioners for help
  • Administer medications as prescribed and in accordance with the NMC under patient Specific Directions (PSD)
  • Deliver opportunistic health care
  • Ensure the privacy, dignity and safety of individuals is always maintained
  • Demonstrate the ability to recognise changing priorities seeking advice and guidance from a registered care professional as appropriate
  • To be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency
  • Report back and share information with the registered care professional on the condition, behaviour, activity, and responses of individuals
  • Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals

  • Demonstrate good understanding of the Mental Capacity Act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from the registered care professional as required
  • Plan and manage competing demands of job role, study, and placement activities
  • Work in an effective and organised manner demonstrating excellent time management and organisational skills to effectively deliver person-centred care for an allocated group of individual
