PCN Enhanced Practice Nurse

Job summary

We are seeking to recruit an part time (25hours) Enhanced Practice Nurse to join our growing multi-disciplinary team. The posts will be hosted by one of the 5 practices, but you will work on behalf of all practices in Sedgefield North PCN. Enhanced Access provision which may include working until 8pm on an evening (one day per week) and possibly Saturdays 9am-5pm on a rotational basis, although given the nature of this role, it is likely working hours will between 8am-6pm.

Main duties of the job

It is a requirement of the role to be registered with the Nursing and Midwifery Council (NMC) and to meet the specific qualification and training requirements the candidate must be educated to masters level or level 7 equivalent or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, women's health and public health and population health management they will have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients.

The successful candidate will benefit from the support of the wider PCN team and have expertise in carrying out long-term condition reviews (including frailty) for our housebound. You will be expected to utilise your skills in clinical assessment and diagnostics to make decisions around patient treatment plans. The successful candidate must be competent in all areas of chronic disease management.

Job Purpose

The purpose of this role is to promote and support improvements to improve the quality of patient care through annual patient reviews in those housebound patients. The Enhanced Nurse will work with all 5 practices within the PCN. The post holder will help patients to get the best from their face-to-face annual reviews including the integration with other PCN services where applicable and supporting aspects of the Quality and Outcomes Framework (QOF).

Key Responsibilities of the Role

Works as part of the practice's MDT.

Consults with patients, their families and the wider multi-professional teams to undertake assessments of patient need and evaluation of complex care plans.

Evaluate problems using clinical knowledge, seeking out and applying relevant evidence, enhanced techniques and interventions to make clinical decisions.

Delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery and innovation.

Advise and teach patients and their families on how to manage their condition.

Participates in and conducts clinical audits, research projects and implements changes as required, including in the development of protocols.

Works within local and national protocols.

Recognises boundaries of their practice and knows when and to whom patients should be referred on to.

Demonstrates initiative and are creative in finding solutions to problems.

Participate in formal training which promotes best practice in the area of expertise.

Advise and support on the development and delivery of workstreams relating to the area of expertise as part of the PCN's strategy.

Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care through dissemination of specialist skills and knowledge.

Contribute to the delivery of local policies, procedures and protocols for the PCN to improve identification, assessment and ongoing support for patients and their carers.

Health and Safety/Risk Management

The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.

The post holder will comply with all relevant Data Protection legislation and Information Governance.
