Job Purpose

The purpose of this role is to promote and support improvements to improve the quality of patient care through annual patient reviews in those housebound patients. The Enhanced Nurse will work with all 5 practices within the PCN. The post holder will help patients to get the best from their face-to-face annual reviews including the integration with other PCN services where applicable and supporting aspects of the Quality and Outcomes Framework (QOF).

Key Responsibilities of the Role

Works as part of the practice's MDT.

Consults with patients, their families and the wider multi-professional teams to undertake assessments of patient need and evaluation of complex care plans.

Evaluate problems using clinical knowledge, seeking out and applying relevant evidence, enhanced techniques and interventions to make clinical decisions.

Delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery and innovation.

Advise and teach patients and their families on how to manage their condition.

Participates in and conducts clinical audits, research projects and implements changes as required, including in the development of protocols.

Works within local and national protocols.

Recognises boundaries of their practice and knows when and to whom patients should be referred on to.

Demonstrates initiative and are creative in finding solutions to problems.

Participate in formal training which promotes best practice in the area of expertise.

Advise and support on the development and delivery of workstreams relating to the area of expertise as part of the PCN's strategy.

Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care through dissemination of specialist skills and knowledge.

Contribute to the delivery of local policies, procedures and protocols for the PCN to improve identification, assessment and ongoing support for patients and their carers.

Health and Safety/Risk Management

The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.

The post holder will comply with all relevant Data Protection legislation and Information Governance.

Special Working Conditions

The post-holder is required to travel independently between practice sites within the PCN and other health and community premises as necessary, to support delivery of PCN clinical services and PCN related business or operational meetings.

This job description is intended to provide an outline of the key tasks and responsibilities only. There may be other duties required of the post-holder commensurate with the position. This description will be open to regular review and may be amended to take into account development within the Primary Care Network. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the network.

Experience

Essential

• Good IT skills.
• Excellent communications skills (verbal and written).
• Excellent organisational skills.
• Ability to work safely under pressure.
• Work as part of a team.
• Have excellent time management and workload prioritisation skills.
• Knowledge of audit and research.
• Demonstrate accountability for delivering professional expertise and direct service provision.

Desirable

• Experience of NHS IT Systems including SystmOne and EMIS.
• Non-medical independent prescriber qualification
• All LTC qualifications/experience.
• Understand clinical governance and risk management.

Qualifications

Essential

• Registration with the with Nursing and Midwifery Council (NMC) and up to date with mandatory training and revalidation.
• A post graduate qualification at level 7 or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, womens health and public health and population health management.
