We have an exciting opportunity for an Advanced Clinical Practitioner to work flexibly across our sites at Windermere Medical Practice and Central Lakes Medical Group.

We have a great opportunity with 35 hours per week split between two locations: 17 hours over 2 days at Windermere Medical Practice, and 18 hours over 2 days at Central Lakes Medical Group in Ambleside. We're also open to sharing this role between two applicants, each working 2 days.
You'll be managing your own caseload and referrals, as well as doing home visits, so having a car and a drivers license is essential. Experience in women's health or chronic disease management would be a bonus.

Main duties of the job
The post holder will work as a member of the Practice Team. The post holder will be responsible for recognising, negotiating and implementing processes to enhance the delivery of care.
Patient centred care will be delivered by assessing patients with undifferentiated and undiagnosed problems; initiating treatment, investigations, advice and management of problems using professional autonomous decision making processes, liaising when appropriate with other healthcare professionals and service providers to ensure safe and effective patient care.
The post holder will contribute to development and delivery of a high quality innovative service to the community through the implementation of evidence based clinical practice, supported by continual professional development and mentorship/supervision.
