The role will require coaching skills, patience, empathy and

Main duties of the job
This role helps people to work on improving their health and
wellbeing, specifically providing practical support with the running of the
Social Care Day Unit and developing other services for the benefit of Alston
Practice particularly delivering young families support and walking groups.
The role will be the centre point for building the volunteer
bank and developing volunteer contacts.
The role will require patient facing work in Alston Medical
Practice GP surgery, community setting or remote visiting.
You will be working with patients to help and encourage them
to live a healthier lifestyle by motivation, support and encouragement, to give
them the knowledge and understanding to manage their lifestyle and health
conditions.
You will take a non-judgemental approach that considers the
whole person in addressing existing issues and encourages proactive prevention
of new and existing illnesses, as well as affecting social concerns, such as
isolation and loneliness.
The post holder will undertake holistic assessments, in the
GP surgery, via telephone or video call or at home if the patient is
housebound, and co-design Health and Well-being Plans with individual patients,
identifying support needs to ensure maximum engagement in improving health and
well-being through face to face and virtual contacts, including within the
individuals own homes where appropriate.

responsible for:
Managing
and prioritising a caseload in accordance with the needs, priorities and any
support requirements of individuals on the caseload
Providing
one-to-one health coaching support for people with one or more long term
conditions, based on what is important to them, with the aim of:
Improving
peoples knowledge, confidence and skills-levels of patient activation
Empowering
people to improve their health outcomes and sense of wellbeing
Preventing
unnecessary reliance on clinical service
Providing
interventions such as self-management education and peer support
Supporting
people to establish and attain goals set by the person based on what is

with the wider practice team and system partners to support the triaging of
referrals that connect people to the right intervention/ community-based
activities which support their health and wellbeing
Working
as part of a multidisciplinary multi agency team to promote Health Coaching and
to be ambassadors for Personalised Care and Supported Self-Management,
modelling the coaching approach in their work
To
attend and contribute to team meetings and events as required by the service
To
support the system they work within to develop pathways that are relevant to
the needs of people who receive health coaching
Provide
support to local community groups and work with other health, social care and
voluntary sector providers to support the patients health and wellbeing holistically
Explore
and support access to a personal health budget, where appropriate, for their
care and support
Utilise
existing IT and MDT channels to screen patients, with an aim to identify those

- Demonstrate local knowledge of the services available in the area covered by the PCN.
- Excellent holistic assessment as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
- Proven skills in collating information and data on community resources and organising these in up-to-date and accessible formats for a range of different service users from various communities.
- Excellent record keeping skills and the proven ability to write comprehensive reports for a variety of stakeholders.
