Are you a compassionate and experienced Registered Nurse looking to make a meaningful difference in your community? Prospect Hospice is recruiting a Community Clinical Nurse Specialist (CNS) to join our expert team providing exceptional palliative and end-of-life care at home and in care settings.

Hours: Full and part-time positions available over 7 days (including Saturdays or Sundays on a rota)

What is the role?

As a Community Clinical Nurse Specialist, you'll play a key role in delivering personalised, dignified, and responsive care to individuals with life-limiting illnesses. You'll manage a complex case load, collaborate with multidisciplinary teams, support families, and provide clinical leadership and mentoring within our community services.

Main duties of the job

Key Responsibilities:

  • Deliver expert palliative and end-of-life care in the community
  • Conduct holistic assessments and manage complex symptoms
  • Support anticipatory care planning and decision-making
  • Act as an advocate for patients and families
  • Educate and influence other health and social care professionals
  • Contribute to clinical governance, quality assurance, and professional development
  • Participate in out-of-hours service provision

Do you have?

  • NMC-registered with relevant post-registration experience
  • Strong knowledge of palliative care and symptom control
  • Excellent communication and clinical decision-making skills
  • Full driving licence and access to a car, prepared to travel throughout our defined catchment area (Swindon, Marlborough, Pewsey, Wootton Bassett, Lechlade, Fairford and Shrivenham)
  • Willingness to undertake advanced specialist training (e.g., PACE, prescribing)

It would also be beneficial if you have:

  • Experience in community nursing or hospice care
  • Non-medical prescribing qualification
  • Formal or informal teaching experience

Prospect Hospice is an independent hospice supporting a community of 300,000 people in Swindon, Marlborough and North Wiltshire, and the villages of Lechlade and Fairford in Gloucestershire.

It is the only dedicated provider of specialist end of life care services within it's locality. The Hospices multi-professionals teams support people in the community, in care homes, at the Great Western Hospital and at the Hospice in Wroughton. Care is also extended to the families and friends of patients.

Our Team

The Community Clinical Nurse Specialist team (CNS) offer advice and support to patients living in their own place of residence, including care homes. Working with other health care professionals including GP's, the team provide oversight and guidance, to ensure best practice for patients and families, keeping the patient at the centre of care and allowing them autonomy in decision making. This highly skilled team support patients and families with managing symptoms, advance care planning and navigating their illness throughout its trajectory.

Main Purpose & Scope

The Community Clinical Nurse Specialist (CNS) is responsible for effectively managing and coordinating a caseload of patients and their families living with specialist palliative and end of life care needs.

The prime purpose is to ensure a flexible, responsive and needs led approach to patient centred care.

A key element will be to actively contribute to the Patient Services clinical governance framework to influence care through participation in education, training, research and audit initiatives.

There is a requirement to provide CNS support in the community setting and throughout the out of hours period.

Scope and Limits of Authority

The CNS is required to plan and organise their own day to day work in collaboration with their team members, and make clinical decisions to support patients with complex and often rapidly changing specialist palliative/end of life care needs.

The CNS is required to identify issues that require onward referral to the multi-professional team and facilitate, where appropriate patient discharge.

The CNS does not have any direct budgetary responsibility.
