Job summary
Are you experienced, clinically driven and want to undertake home visits in an Out of Hospital service? The aim of the home
visiting shifts is to support and care for people to stay at home, when it is
clinically safe to do so, to prevent avoidable hospital admissions and escalate
to emergency services other services when clinically needed. This role will
involve working
as an
expert practitioner, demonstrating advanced knowledge and clinical skills in undertaking a holistic assessment, prescribing where appropriate, making
onward referrals, and working autonomously.
We are looking for Advanced Clinical Practitioners
(ACPs) to be contracted on a permanent basis. The shifts available are.
Saturday 12.00 - 21.00
Sunday 12.00- 20.00
Minimum requirement is to cover one shift per week.
Main duties of the job
ACP is an experienced clinician who, acting within
their professional boundaries, will work flexibly to provide care for the
presenting-patient, from initial history taking, clinical assessment,
diagnosis, treatment to evaluation of care. The clinician in this role will be
a key to supporting the clinical work across Mastercall, whilst acting as a
positive and enthusiastic role model, supporting junior staff, and developing
our workforce.
The post holder will demonstrate safe, clinical
decision-making and expert care, including assessment and diagnostic skills,
for patients within the out-of-hours, supporting new work streams and services
to manage patients out of hospital. The post holder will demonstrate critical
thinking in the clinical decision-making process. They will work
collaboratively and flexibly with the wider healthcare team to meet the needs
of patients, and at times using their own initiative.
Mastercall ACP's will undertake
remote clinical assessments as well as face to face depending on the needs of
the service and will be encouraged to participate in service development and
improvement for the benefit of patients and/or the
organisation, through identifying opportunities to innovate and work
differently to meet the demands of the changing healthcare environment. In
order to work at this level NMC/ AHCP requirements for advanced practice must
be met
