Main duties of the job
Locala is welcoming applications from enthusiastic, motivated, and caring Registered nurses to join our Community Nursing Network teams. Whether you are newly qualified or have experience, we will support you to thrive in your role.
We have nine teams across North Kirklees and Greater Huddersfield, delivering high-quality care to patients in their own homes and local communities. As a Band 5 Community Staff Nurse, you will play a vital role in providing person-centred care, carrying out holistic assessments, and supporting patients to manage their health needs effectively.
Your responsibilities will include:
- Delivering seamless nursing care to housebound patients.
- Coordinating care to prevent unnecessary hospital admissions.
- Promoting self-care for patients with long-term conditions.
- Contributing to health promotion and innovative projects that improve patient outcomes.
- A qualified nurse with energy, enthusiasm, and a commitment to high standards of care.
- Excellent communication and interpersonal skills.
- Adaptability, reliability, and a positive approach to teamwork.
