Main duties of the job
We have an excellent opportunity for a Band 6 Dietitian to join our Community Dietetics Team covering Greater Huddersfield.
We are looking for someone who can work as a specialist, autonomous dietitian with experience in managing malnutrition in the community. You should have strong organisational, communication and leadership skills and embrace the challenge of service development.
You will be responsible for providing comprehensive nutritional care for patients referred to our service, offering assessments, in clinics, homes and virtual appointments, working alongside peers, senior colleagues, a dedicated Band 4 assistant and with other AHP's including speech and language therapists.
The role supports patients at risk of malnutrition, adopting a food first based approach with the appropriate use of oral nutritional supplements.
You will be responsible for:
- Your own caseload of patients who require oral nutrition support in the community offering holistic person-centred care Joint working with Speech and Language therapist and other AHP's, Social care and Nursing colleagues.
- Offering a hybrid approach including telephone/video clinics and home visits as needed.
- Supporting student placements (A, B and C placements)
- Planning and delivering training (internal and external)
The post requires a minimum of 2 years post graduate experience and youmustbe able to fulfil the travel requirements of the post.
