We are seeking enthusiastic, motivated and reliable nurses who are keen to support and deliver a range of quality nursing services to our patients within Kirklees. You will have the energy, enthusiasm and drive to support our teams to deliver high quality services, as well as excellent professional skills.

Working within the Community nursing teams, you will undertake duties involving delivery of nursing services and holistic assessments of patients health needs using a person-centered approach including providing quality and seamless nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

In addition, you will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion, and complex out of hospital patient care.

Frailty and OPAT Team Competencies

  • To provide holistic nursing assessments
  • To follow the escalation plan for the deteriorating patient
  • To provide patient centred care
  • Assume delegated responsibility for holistic assessment and the planning, implementation, monitoring and evaluation of programmes of care/ care packages, contributing to complex assessment/ care packages.
  • To help the patient towards self-care and maximising independence, maintain their optimum level of health and prevent further deterioration.
  • To delegate work appropriately within the competency framework and to take responsibility for the workload as required for short periods with support from a designated Band 6 or clinical lead.
  • To provide high quality evidence-based nursing practice to patients within their own home.
  • Deliver the Outpatient Antibiotic Pathway (OPAT)
  • To support the role of the OPAT Clinical Lead.
