We are seeking a highly motivated District Nurse with the aptitude, motivation, and leadership skills, to drive local changes and integration within community nursing. As a named District Nurse for a group of GP practices, accountable for a caseload of patients, you will operationally manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.

You will have the drive to motivate and lead the team and actively case manage the patient caseload across the service. You will also be involved in the implementation of digital technology to aid patient care and support the vision.
