Cardiac Rehabilitation Nurse Specialist
Band 6: £35,392 - £42,618
33.5 hours per week

We are looking for a motivated and enthusiastic individual to join Locala's Cardiac Rehabilitation service with the existing team of experienced and dedicated professionals of Cardiac Nurse Specialists, Cardiac Specialist Physiotherapist, Exercise Physiologist, Health Care Assistant and Rehab Assistant to work as Cardiac Rehab Nurse Specialist Nurse role.
Main duties of the job
You will be providing care and management for patients diagnosed with coronary conditions and have had a cardiac intervention, supporting with lifestyle risk assessments, education around their cardiac event and working with the Therapy team in delivering tailored exercise and activity programs. You will work with our dedicated Locala Cardiology Specialist team as part of the service in Kirklees in Huddersfield.
We are looking to employ an experienced nurse with cardiology and cardiac rehab background, but If you are an experienced nurse and have a passionate interest and knowledge in cardiac conditions and exercise and activity, we would look at supporting a developmental role for Band 5 nurses who have leadership potential and are dedicated in providing motivational management and supporting patients with their cardiac condition.
Responsibilities:
- Utilising clinical assessment skills to provide cardiologyassessment of patients with complex needs.
- Utilise motivational communication techniques to assist in identifying lifestyle risks and assisting in behavioural change when indicated and maximise independence where possible.
- Contribute and implement care plans for patient and cares and other services including social care as appropriate.
- Co-ordinating and overseeing secondary prevention treatment
- Delivering community clinics and home visiting service
- Delivering cardiac rehabilitation exercise program, including delivery of educational sessions.

- Act as a resource for advice and education within the primary care environment.
Skills/Education:
- Educated at or working towards degree level qualification.
- Post registration experience in specific cardiac conditions or willingness to gain experience.
- Experience/good knowledge of working in primary care.
- Worked with patients and significant others to support personal change management strategies.
- Evidence of good communication skills.
- Qualified registered nurse.
- Current NMC registration.
- ENB 998 or equivalent or working towards.
