Diabetes Support Nurse
Development Role: Band 5/6 - £28,407 - £42,618

An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Locala Diabetes Service.
Ideally we are seeking the skills of a Band 6 Diabetes Specialist nurse who can make a difference to the health and wellbeing of individuals referred to the service. However this is also open as a band 5/6 development role.
The role requires a motivated individual who is interested in developing themselves and others to deliver high quality, effective and innovative diabetes care in the community.
You will need to demonstrate knowledge of national and local issues and be committed to partnership working with other providers.
As part of a team and working in conjunction with GPs, mainstream nursing, social and secondary care services, you will provide a patient focused, co-ordinated case management approach across primary care.
The post holder will participate in and influence efforts across health and social services to shape the service, to support patient choice, improve quality of life, and promote self-management whilst adopting the community care team approach.
