Ageing Well Practitioner

Main duties of the job
Are you a ANP or matron of the future? Do you have the passion to gain skills in complex case management, LTC and Frailty?
Locala have undertaken an innovative community nursing redesign, creating a structure of community nursing networks to improve delivery of effective high-quality care incorporating frailty, Ageing well and Integrated Care Systems agenda, aligning and interdependent with primary care networks.
We need dynamic, highly motivated practitioners for our Ageing Well Department.
As a Ageing Well Practitioner you will deliver the excellent service, and continuously improve this role, by working alongside a team of highly skilled ANP and matron colleagues, supporting frail patients with Complex Long Term conditions within our community.
You will also have the opportunity to work alongside the Acute services, primary care, social care and voluntary sector within an MDT to develop pathways to ensure acutely unwell and frail patient needs are met.
You will need to be a key communicator and develop effective relationships with stakeholders, support specialist case management and manage a virtual ward integrated within the community nursing networks.
