Band 3 Health Care Support Worker
Kirklees Virtual Ward
Hours per Week: 15 hours7 day service

Main duties of the job
The Role:
Are you looking for a new, exciting challenge? The new Virtual Ward service was commenced from October 2022. This service will support patients with acute Frailty needs who will have their acute care provided in the community instead of having their care within a hospital setting. A full valid UK driving licence with access to a vehicle and business insurance is essential for the role.
We are seeking several Health Care Support Workers to join our new team with an interest in acute support for Frailty patients within the new service, working alongside our colleagues across Calderdale, Kirklees and Wakefield.
The Virtual Ward service offers acute care at home to Kirklees residents 7 days a week between the hours of 8am and 8pm.
The patients will be supported across Kirklees by a dedicated team, with oversight and support from Acute Consultants / appropriate clinicians.
As a key member of the Virtual Ward, youll be part of our continuing success in Kirklees, preventing admission to hospital and supporting earlier discharge by helping keep patients at home. The service works as part of a Calderdale, Kirklees, and Wakefield (CKW) approach.
The Virtual Ward service is a multidisciplinary team with Locala colleagues including Frailty Nurses, Health Care Support Workers, and an Administrator. They will work in partnership with clinicians within the local Acute Trusts to ensure patients have a holistic and safe service offer.
